OFFICE POLICY

 

CANCELLATION POLICY:  Please provide at least 24 hours notice if you need to cancel or reschedule your appointment.  Please contact me by phone or email in order to avoid late cancellation or 'missed appointment' charges.  These fees are billed directly to you and cannot be submitted for insurance reimbursement.  The 24 hours advanced notice gives me enough time to contact another client and offer them the available time slot.   Your consideration is greatly appreciated.

HIPAA POLICY

 

PRIVACY POLICY: The privacy of your treatment information is extremely important.

All records are maintained through a fully compliant electronic medical record system. The Health Insurance Portability and Accountability Act passed by Congress in 1986 stipulates under what circumstances your health information (PHI) can be shared or disclosed.  I will provide you with copies of the policies concerning HIPAA practices at your request.
